Job Summary
Manage patients with emotional and health problems efficiently as part of the primary care team. Provide timely interventions, transition patients with chronic behavioral conditions to appropriate services, and link families to resources. Maintain records documenting interventions per program requirements and participate in peer evaluation and training. Monitor the behavioral health program to identify patient care problems and recommend improvements. Prepare basic correspondence, reports, and presentations using Microsoft Office tools while entering data into centralized electronic health records. Uphold confidentiality regarding patient health information and clinic policies.
Required Qualifications
- Possesses an understanding of the application of behavioral health theory, case management interventions, and ethical practices of the behavioral health profession.
- Requires the equivalent of extensive formal training in providing behavioral health and case management interventions, usually acquired in the completion of a Master's Degree with a major in social work, counseling, or related field.
- Must be bilingual/biliterate in English/Spanish
- Possesses a license to provide behavioral health interventions/counseling (LMHC, LMHC-A, or LICSW)
- Possesses a valid driver's license.
- Ability to prepare basic correspondence and simple reports in Microsoft Word.
- Ability to use Microsoft Excel to create tables and simple displays of information.
- Ability to create basic presentations in Microsoft PowerPoint.
- Ability to enter and manage data into a centralized electronic health records
