Job Summary
Behavioral Health Consultant provides behavioral health services within a fully integrated primary care setting, collaborating with an interdisciplinary team to manage psychosocial aspects of chronic and acute diseases, conduct brief evidence-based interventions, develop clinical pathways, and support patients in achieving self-management and health goals; responsibilities include consultation and co-management of mental/behavioral health issues, crisis triage, and ensuring seamless integration across departments.
Required Qualifications
- Master's degree in Behavioral Health (Psychology, Social Work)
- Two years of directly related experience
- Certification or certification eligibility as an LCSW, Ed.D, Psy.D, LPC or Ph.D
- Certification to perform CPR and AED per AHA/Red Cross guidelines
- Level One Fingerprint clearance card issued by Arizona Department of Public Safety
- Computer literacy
