Job Summary
Cultivate strategic relationships with courts, schools, faith centers, and healthcare providers to establish referral pathways for ACC's behavioral health services. Conduct regular outreach visits, deliver educational presentations to partners, and manage the full referral intake process from initial contact through case assignment. Track referral metrics in the Birdseye Database, conduct community needs assessments, and maintain active case management caseloads including assessments, service plan development, and client advocacy. Provide crisis support and coordinate linkages to housing, transportation, and peer resources while documenting progress in the electronic health record. Requires valid driver's license for county-wide travel across Oakland, Wayne, and Macomb Counties.
Required Qualifications
- Bachelor's degree in social work, psychology, counseling, public health, or a related human services field
- Minimum 2 years of experience in behavioral health, social services, or community health outreach
- Demonstrated knowledge of behavioral health conditions and community resources
- Strong interpersonal skills and ability to establish rapport across diverse community stakeholders
- Valid driver's license and reliable transportation
- Proficiency with EHR systems and standard office software
Desired Qualifications
- Experience working with justice-involved individuals, school-based programs, or hospital and emergency department settings
- Bilingual fluency (Arabic or other languages reflective of the local population served)
- Experience with Medicaid and managed care referral and authorization processes
