Job Summary
Develop person-centered treatment plans to resolve presenting difficulties and support clients in transitioning to less intensive community settings. Provide direct service leadership when management is absent, empowering clients to meet goals and access resources for housing, medical care, and food. Coordinate intakes, respond to referrals, and ensure admission eligibility criteria are met with proper documentation. Maintain client records, progress notes, and data entry according to program standards while developing activities that promote wellness. Deliver on-the-spot counseling, family interventions, and crisis management, including after-hours rotational coverage. Facilitate family support meetings to aid client transitions into natural support systems.
Required Qualifications
- Master's degree in Social Work or related field
- Minimum of 2 years' experience as a provider of mental health services
- Valid registration with the Board of Behavioral Sciences (BBS)
- California driver's license
- Current vehicle insurance/registration
- Reliable means of transportation capable of passing vehicle safety inspection
