Job Summary
Coordinate care and manage case loads by implementing support plans, monitoring medications, and facilitating individual planning processes for clients in residence, community, or agency settings. Provide one-on-one services using a personal vehicle for medical appointments and community resource navigation, while actively listening to encourage positive behaviors and social interaction. Manage flexible schedules, handle emergency situations with a calm demeanor, and maintain safe environments by adhering to all policies and procedures. Process required documentation within timeframes and obtain 50% billable time. Work closely with the Behavioral Health Services team, guardians, and family to assess progress and needs, ensuring compliance with mandated reporter procedures.
Required Qualifications
- High School Diploma and/or GED
- One (1) year experience working with or having direct life experience with individuals who experience mental illness
- Basic computer use and experience with Microsoft Office
- Background checks as defined by licensing agencies
- Current TB test/clearance
- Valid driver's license and proof of insurance
- Clean driving record
- Pass mandatory agency trainings (i.e., CPR, Mandt, Programs)
- Completed all departmental trainings
- Ability to manage a flexible and changing schedule
- Ability to apply common sense and effective problem-solving skills as appropriate
- Ability to handle emergency situations by communicating the appropriate information to designated people and maintaining a calm and professional manner
- Demonstrates cooperative and effective professional behavior with colleagues and supervisors in the course of work
- Ability to gather all information necessary to make appropriate decisions independently
- Ability to interpret instructions in written, oral or schedule form
- Ability to see within normal parameters
- Ability to hear within normal range
- Communication skills using the English spoken word
- Must be located in Alaska to be eligible for an interview
Desired Qualifications
- Preferred, associate degree in Social Work, Human Services, or related field
- Three (3) years' experience working with or having direct life experience with people who experience mental illness
