Job Summary
Assess member needs across physical, mental, and social health domains to coordinate care plans and facilitate access to services. Connect clients to community resources, advocate with healthcare providers, and arrange transportation while utilizing evidence-based practices like Motivational Interviewing. Conduct outreach to facilitate program linkage, log activities in the CRM system, and evaluate client progress against SMART goals. Complete all documentation, maintain electronic health records, and submit monthly compliance reports. This hybrid role requires traveling up to five days weekly within a 20-mile radius of Riverside County, CA, with a Monday-Friday 8:30am-5:00pm schedule. Candidates must hold an active California social work or counseling license and be bilingual in English and Spanish.
Required Qualifications
- Active, unrestricted ACSW, LCSW, LMFT, or LPCC license in California
- 1+ year of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely, with flexibility for potential evenings and weekends
- Working knowledge of government and community resources related to social determinants of health
- Excellent oral and written communication skills
- Positive interpersonal skills
- Valid driver's license and reliable transportation
- Must have general computer skills and a working knowledge of Google Workspace, MS Office, and the internet
- Bilingual (English/Spanish)
- Must reside within 20-miles of the assigned territory
- Must be vaccinated against influenza
- Compliance with federal law requires identity and work eligibility verification using E-Verify upon hire
