Job Summary
Provides a wide range of direct and indirect case management services to clients with severe and persistent mental illness, including assessment of strengths, abilities, needs, desires, preferences, referrals and coordination of services. Services are provided in the office, home, and various locations in the community. Performs crisis intervention as needed, coordinates care with social service providers and other team members, develops and implements solution-focused interventions, identifies treatment options, assists clients with linking to resources, and maintains thorough documentation and participation in quality/utilization management activities. Requires a Bachelor’s Degree (or equivalent experience), willingness to travel, and is supported by a comprehensive benefits package; bilingual skills are a plus.
Required Qualifications
- Bachelor's Degree in Psychology, Social Work, Sociology or related field and prior experience required
- An Associate's Degree (or at least junior standing in a Bachelor's level program) in Human Services, Psychology, Social Work or related field and at least 3 years of full-time employment experience working directly with individuals with severe and persistent mental illness will be considered in lieu of the Bachelor's Degree
Desired Qualifications
- Bachelor's Degree in Psychology, Social Work, Sociology or related field
- An Associate's Degree (or at least junior standing in a Bachelor's level program) in Human Services, Psychology, Social Work or related field with at least 3 years of full-time employment experience will be considered in lieu of the Bachelor's Degree
- Experience in the field of mental health is preferred
- Basic computer skills including Windows and Microsoft Office
- Valid Ohio driver's license with clean driving record and proof of adequate personal automobile insurance
- Knowledge of Dialectical Behavior Therapy is a plus
- Bilingual (English/Spanish) a plus
- Experience with EHR (Electronic Health Records) is a plus
