Behavioral Health Case Manager

Provide case management and follow-up care to support patients in meeting health and treatment goals. Collaborate with primary care and behavioral health providers to coordinate care, treatment plans, and patient education related to mental health, substance use, and psychosocial needs. Serve as a liaison across the integrated care team to improve communication, assess social determinants of health, and connect patients to appropriate internal and community resources. Conduct secondary suicide risk assessments, develop safety plans, and assist with crisis de-escalation using trauma-informed approaches. Facilitate integrated care clinical consult team meetings and mentor students training at Noble. Work a 10-hour shift, four days a week, with no weekends, while advocating for underserved communities within Manitowoc, Sheboygan, and Washington Counties.
