Job Summary
Behavioral Health Case Manager II in Ohio — perform telephonic and home-visit-based case management for members with behavioral health and substance use disorder needs within licensure scope; engage in complex case assessments, develop and monitor care plans, coordinate resources and providers, support access to quality care, and collaborate with cross-functional teams. Requires MA/MS in relevant field or nursing degree, active Ohio license, 3+ years of clinical experience, and state licensure eligibility across allowed licenses; hybrid/virtual work with mandatory onsite weekly requirements; Ohio-licensed professionals eligible for in-state roles; salary range $69,620-$104,280; locations include Columbus, Cincinnati, Seven Hills, Mason with hybrid work arrangement.
Required Qualifications
- MA/MS in social work, counseling, or related behavioral health field OR nursing degree
- Current active unrestricted license (RN/LCSW/LMHC/LICSW/LPC/LMFT/LMSW or Clinical Psychologist) to practice within scope of licensure in applicable states
- Minimum 3 years of clinical experience in social work counseling with broad range of complex psychiatric and substance use disorder treatment
- Previous experience in case management and telephonic and/or in-person coaching with members with complex psychiatric/substance abuse/medical disorders
- Managed care experience
- State of Ohio licensure is necessary
