Job Summary
Behavioral Health Case Manager II responsible for telephonic and home-visit case management for members with behavioral health and substance use needs; acts as subject matter expert in areas such as Substance Abuse or Mental Health Disorders; conducts assessments, develops and monitors care plans, coordinates with internal/external resources, and supports access to quality, cost-effective care; requires an MS/MA in a behavioral health field and an active health-professional license; role involves virtual full-time work with in-person training and hybrid/onsite requirements.
Required Qualifications
- Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing
- Current active unrestricted license such as RN LCSW LMHC LICSW LPC LMFT LMSW or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required
- Minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
- Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders
- Managed care experience required

- Experience in health coaching and motivational interviewing techniques preferred
