Job Summary
Field-based Behavioral Health Case Manager in Missouri who conducts telephonic and in-person client visits to develop and monitor care plans for members with behavioral health and substance use needs. Responsibilities include conducting assessments to identify needs, creating care plans, monitoring effectiveness, coordinating with internal and external resources, and ensuring access to quality, cost-effective care. Requires MA/MS in social work, counseling, or related behavioral health field (or nursing degree), 3+ years of clinical experience, and current licensure (e.g., LCSW, LMHC, LPC, LMFT, RN, or Clinical Psychologist). Position supports hybrid work with weekly onsite requirements under Elevance Health's Hybrid Workforce Strategy; vaccination policy may apply.
Required Qualifications
- MA/MS in social work, counseling, or related behavioral health field or a degree in nursing
- minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse/substance abuse disorder treatment
- Current active unrestricted license (e.g., RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist) to practice as a health professional within scope of licensure in applicable states
- Ability to conduct case management telephonically and/or via home visits within licensure scope
- Reside in Missouri; candidates residing in Columbia or Springfield or Kansas City MO area encouraged to apply
