Job Summary
Behavioral Health Case Manager I manages and coordinates care for members with behavioral health and substance use needs, conducting telephonic and home-visit assessments, developing and modifying care plans, monitoring effectiveness, and coordinating with internal/external resources to ensure access to quality, cost-effective care; role requires licensure in applicable state(s) and compliance with hybrid/virtual work policy, including occasional in-person training and required site visits.
Required Qualifications
- MA/MS in social work, counseling, or related behavioral health field or a nursing degree
- minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse treatment
- Current active unrestricted license such as RN LCSW LICSW LPC LMHC LMFT LMSW or Clinical Psychologist
