Behavioral Health Case Manager I

Job Summary

Conduct behavioral health case management telephonically and via home visits for members with substance abuse and psychiatric disorders. Assess member needs using clinical judgment, develop care plans to address identified objectives, and monitor plan effectiveness with modifications as required. Coordinate with internal and external resources to support member access to quality, cost-effective care. Requires a Master's degree in social work, counseling, or nursing with three years of clinical experience, plus an active unrestricted license (e.g., LCSW, RN, LMHC) in Virginia. Full-time virtual work with approximately 20% field-based components, Monday through Friday, 9:00am to 5:00pm.

Required Qualifications

  • MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing
  • minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
  • any combination of education and experience which would provide an equivalent background
  • Current active unrestricted license such as RN LCSW (as applicable by state law and scope of practice) LMHC LICSW LPC (as allowed by applicable state laws) LMFT LMSW (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States
  • within the state of Virginia
  • licensed in VA
  • Current active unrestricted license such as RN LCSW (as applicable by state law and scope of practice) LMHC LICSW LPC (as allowed by applicable state laws) LMFT LMSW (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States
  • LAPC LAMFT (as allowed by applicable state laws)
  • any other state or federal requirements that may apply
  • current PR health certificate and a current PR Law 300 certificate

Desired Qualifications

  • Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders
