Job Summary
Facilitate residents' linkage with community services including case management, clinical support, and vocational opportunities. Develop comprehensive needs assessments, conduct client meetings to review progress, and provide daily living skills training in medication monitoring, cleaning, and money management. Deliver individual counseling, support services, and medication management while engaging clients in recovery planning. Maintain accurate documentation for client records and ensure compliance with all agency requirements. Attend supervision and staff meetings. Bachelor's degree preferred with two years of experience in mental health settings.
Required Qualifications
- Bachelor's Degree preferred in Psychology, Rehabilitation, Social Work or other related mental health discipline
- 2 years' experience working in a similar setting
- experience working with people with psychiatric disabilities
- good verbal and written communication skills
- ability to make sound judgments regarding client care
