Behavioral Health Care Manager - Licensed

Job Summary
Conduct comprehensive assessments and develop person-centered care plans for high-need Medi-Cal members as the primary point of contact for their behavioral health and psychosocial needs. Coordinate care across behavioral health providers, medical teams, and community resources while performing in-person home and community visits to support transitions of care and post-discharge follow-up. Provide education, coaching, and trauma-informed support to ensure meaningful, in-person member engagement within a hybrid schedule of 2–3 days in the field and 2–3 days in the office. Maintain accurate documentation and meet program requirements for the Enhanced Care Management initiative.
Required Qualifications
- Master's degree in Social Work or Psychology
- Active Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Clinical Counselor
- 1+ years in care management, behavioral health, or related setting
- Valid driver's license, insurance, and reliable vehicle
- Comfortable with field work and home visits
Desired Qualifications
- Medi-Cal/ECM experience
- Bilingual English/Spanish
- Strong communication and care coordination skills
- Knowledge of behavioral health, SDOH, and community resources
