Behavioral Health Care Manager

Job Summary
Behavioral Health Care Manager coordinates and manages care for patients with behavioral health conditions, develops individualized care plans with behavioral health providers, patients, families, and community partners, conducts mental health assessments using PHQ-2/PHQ-9, ensures compliance with Joint Commission standards, PCMH requirements, and HIPAA, and oversees care coordination, crisis support, referrals, follow-up, and outreach to improve health outcomes. The role involves collaboration with hospitals, schools, and community agencies, documentation in EMR, patient engagement, care transitions, and continuous quality improvement, with ability to travel between clinic locations as needed.
Required Qualifications
- Bachelor’s degree in social work, Psychology, Counseling, Public Health, Human Services, Behavioral Sciences, or a related field
- Minimum of three (3) years of healthcare, behavioral health, care coordination, case management, community-based services, or related patient service environments
- Experience working with individuals experiencing mental health conditions, substance use disorders, and psychosocial barriers to care
- Current CPR/BLS Certification required (preferred additional certifications)
Desired Qualifications
- Bachelor’s degree in social work, Psychology, Counseling, Public Health, Human Services, Behavioral Sciences, or a related field (required); Master’s degree preferred
- Minimum of three (3) years of healthcare/behavioral health care coordination or related patient services experience
- Experience with mental health conditions, substance use disorders, and psychosocial barriers to care
- Knowledge of Joint Commission standards, PCMH, and HIPAA; EMR proficiency; strong communication and organizational skills
