Job Summary
Connect patients and families to appropriate mental health and community resources for long-term care, coordinating with professionals, schools, and families to reduce barriers and improve satisfaction. Lead mental health care coordination efforts by overseeing other coordinators, assigning work, and monitoring project completion while developing internal referral processes and training new Care Coordinators. Manage direct service referral pathways, collect and analyze data for assigned programs, and prepare monthly reports for the Texas Child Mental Health Care Consortium. Ensure clinical documentation compliance within secure systems like REDCap and Athena, serving as a liaison for data projects. This grant-funded role at Dell Medical School supports the Texas Child Mental Health Care Consortium and Dell Children's Medical Center to improve mental health care accessibility for children and adolescents across Texas.
Required Qualifications
- Bachelor's Degree in Social Work, Psychology, Education, or a related field
- at least 5 year(s) of experience in a health care or educational setting
- experience preferably in K-12
- Proficiency in electronic systems including, but not limited to, Google Suite, Trayt, Slack, Athena or Epic, Calendly
Desired Qualifications
- Master's Degree in Social Work, Psychology, Education, or a related field
- Knowledge of managed care, medical terminology, patient referral process, claims, and CPT coding
- Experience working with pediatric behavioral and mental health patient populations
- Knowledge of educational practices supporting mental health and social emotional learning in K-12 settings
- Experience developing, refining, and evaluating patient care plans or student education plans
- Bilingual (English/Spanish) language proficiency
