Job Summary
Coordinate care for Spirit of Healing members and NARA NW patients by administering mental health assessments, treatment plans, and referrals to community programs. Collaborate with multidisciplinary teams and providers to develop care goals, facilitate communication between clients and medical staff, and attend appointments as needed. Identify and reduce barriers to care using a culturally sensitive, person-centered approach that respects client autonomy and employs motivational interviewing principles. Document all activities in the Electronic Medical Records system and provide timely updates on self-management strategies and specific member goals.
Required Qualifications
- Bachelor's degree or higher in behavioral health, social services, psychology, or a related field
- Minimum 3 years of experience in behavioral health, care management, care
- Active QMHA-I or QMHP-C through MHACBO
- Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
- Ability to operate a motorized vehicle
- Demonstrated word processing and computer skills, including ability to learn new systems/databases
- Minimum 2 years of electronic health record experience
- Ability to maintain professional boundaries
- Must pass a pre-employment or for-cause drug tests
- Must pass criminal background and DMV checks
- Must be able to lift 50 lbs
Desired Qualifications
- Active Oregon license (LCSW or LPC) or ability to obtain by start date
- Experience with utilization of motivational interviewing and trauma informed care
- Specialized training in Evidence Based Practices providing behavioral changes
- Experience working with diverse populations that have experienced obstacles and barriers related to marginalization
- Experience working with Tribal communities and intergenerational trauma
- Knowledge of or willingness to learn gender-affirming care needs, available resources, and the transgender experience
- Experience working within the Native American/Alaska Native community
