Job Summary
Provide case management, coordinate care, and advocate for children, adolescents, and families through community-based behavioral health services. Collaborate with multidisciplinary treatment teams to develop and evaluate care plans, connect clients to essential resources and services, coordinate referrals to community organizations, maintain accurate documentation, and work across multiple locations with flexible hours in a hands-on, client-facing role.
Required Qualifications
- Bachelor’s degree in a Human Services discipline from an accredited college or university
- Experience working with children and/or adolescents in a therapeutic, community-based setting
- CPR and First Aid certification (or willingness to obtain prior to working with clients)
- Strong organizational, communication, and teamwork skills
- Ability to work flexible schedules and travel between service locations
