Behavioral Health Care Coordinator

Job Summary
Coordinate behavioral health services in SNFs and office environments, travel within the assigned region to provide onsite support, and facilitate service coordination across Psychiatry, Therapy, and Neuropsychology. Responsibilities include referral assistance, consent management, staff education, attending facility and MD meetings (GDR, QAPI), ensuring PASRR II compliance, scheduling in-services, and Telehealth support. Requires strong organizational, communication, and relationship-building skills, proficiency with technology and EHR (Athena), and a minimum high school diploma with 3-5 years of experience in skilled nursing facilities. Must be vaccinated for COVID-19 and capable of working in both office and healthcare settings.
Required Qualifications
- Minimum of a High School Diploma or GED
- Some college education preferred, focus on behavioral health or psychology
- 3-5 years of experience in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator
- Proficient in Windows environment, Internet, MS Office (Outlook, Word, Excel)
- Proficient in EHR software systems (Athena) preferred
- Ability to travel to SNF facilities within the assigned region
