Behavioral Health Care Coordinator

Behavioral Health Care Coordinator to travel within Missoula and Helena, MT, to provide onsite behavioral health services in skilled nursing facilities and coordinate care with facility staff and clinicians. Responsibilities include coordinating psychiatry, therapy, and neuropsychology services; assisting with referrals and consents; educating facility staff on processes; attending behavior management meetings and coordinating MD attendance for facility meetings; ensuring PASRR II compliance; scheduling in-services; facilitating Telehealth visits; supporting facility onboarding; and performing additional duties as assigned. Key skills include strong communication, organizational abilities, relationship-building with facilities and providers, proficient use of PCs and EHR systems (Athena), and knowledge of behavioral health terminology and documentation practices. Required travel to SNFs, ability to work in office and clinical settings, and reliability in a fast-paced healthcare environment. Current COVID-19 vaccination per safety protocols. Minimum 3-5 years in skilled nursing facilities with a favorable track record; strong documentation and collaboration capabilities.
