Job Summary
Behavioral Health Care Coordinator responsible for coordinating and delivering essential behavioral health services to patients in skilled nursing facilities and during field visits. Travel within the assigned region to provide onsite support, collaborate with facility staff (Executive Directors, Directors of Nursing, case managers, social workers, admissions personnel, surveyors, physicians), coordinate psychiatry, therapy, and neuropsychology services, assist with referrals and consents, educate facility staff on processes, attend behavior management meetings, ensure PASRR II compliance, schedule in-services, and facilitate telehealth visits to enhance patient care. Must build positive relationships with facilities, manage documentation and data-driven tasks, and adapt to a fast-paced healthcare environment while maintaining vaccination requirements and proficiency with office software and EHR systems.
Required Qualifications
- High School Diploma or GED
- Some college education preferred with focus on behavioral health or psychology
- 3-5 years of experience in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator
- Proficiency with PC, Windows, Internet, MS Office (Outlook, Word, Excel)
- Experience with EHR systems (Athena) preferred
