Job Summary
Behavioral Health Care Coordinator responsible for on-site and telehealth coordination of behavioral health services in skilled nursing facilities within assigned regions. Facilitates coordination of Psychiatry, Therapy, and Neuropsychology services; assists with referrals and consent management; educates facility staff on processes; participates in meetings such as GDR and QAPI; ensures PASRR II compliance; schedules in-services; supports Telehealth visits; collaborates with facility staff, directors, case managers, social workers, and physicians to optimize patient care. Requires travel to SNF facilities, strong organizational and communication skills, and proficiency with EHRs (Athena) and MS Office; minimum HS diploma or GED with 3-5 years of relevant experience; vaccinated per safety protocols; benefit offerings include competitive earnings, PTO, 401k match, and healthcare coverage.
Required Qualifications
- Minimum of a High School Diploma or GED
- Some college education preferred with focus on behavioral health or psychology
- 3-5 years of experience in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator
- Proficient in PC use, Windows environment, Internet, and MS Office Applications (Outlook, Word, Excel)
- Proficient in EHR software systems (Athena) preferred
- Current COVID-19 vaccination per safety protocols
