Job Summary
Behavioral Health Care Coordinator coordinates and delivers behavioral health services in skilled nursing facilities and office environments, traveling within the assigned region to provide onsite support. Responsibilities include coordinating Psychiatry, Therapy, and Neuropsychology services; assisting facilities with referrals and consent management; educating facility staff on processes; attending and coordinating facility meetings (GDR, QAPI); ensuring PASRR II compliance; scheduling in-services; facilitating Telehealth visits; onboarding facilities with Business Development; and performing additional duties as assigned. Must maintain strong customer service, organizational, and communication skills, and have 3-5 years of relevant experience with familiarity in EHR (Athena) and MS Office; current COVID-19 vaccination. Minimum education is High School Diploma or GED, with some college preferred in behavioral health or psychology; willingness to travel to SNFs within the community. Competencies include survey process knowledge, behavioral health expertise, multitasking, problem-solving, and relationship-building with facilities, providers, and supervisory teams.
Required Qualifications
- High School Diploma or GED
- Some college preferred with focus on behavioral health or psychology
- 3-5 years of experience in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator
- Proficient with Windows OS, Internet, MS Office (Outlook, Word, Excel)
- Proficient with EHR systems (Athena) preferred
- COVID-19 vaccination per safety protocols
- Ability to travel to SNF facilities within assigned region
- Strong organizational, communication, and relationship-building skills
- Ability to educate facility staff on processes and coordinate multiple services (Psychiatry, Therapy, Neuropsychology)
- Compliance with PASRR II requirements
- Ability to use telehealth to enhance patient care
