Job Summary

Identify high-risk patients, implement crisis intervention practices, and maintain a safe environment through safety plan techniques. Perform environmental and patient safety rounds, report observations to nursing staff, and assist in assessments for restraint application and removal. Lead discussion groups on anger and safety, participate in debriefing after codes or incidents, and review incident reports to identify preventable factors. Escort patients during transfers, testing, or admission, and assist staff with activities of daily living, lifting, and positioning. Provide direct physical care and one-to-one observation to selected patients under supervision while obtaining vital signs in accordance with hospital policy. Participate in shift hand-offs, unit rounds, and quality assurance activities, and provide ongoing training in de-escalation and physical interventions.

Required Qualifications

  • High School Diploma or GED

Desired Qualifications

  • Associate's degree or some college coursework in psychology, social work, or social services field
  • Prior experience in a behavioral health setting
  • At least 2 years of experience in a direct care role
