Job Summary
Provide care coordination services to patients at risk for health deterioration, social determinants of health issues, sentinel events, or poor outcomes under the direction of the Behavioral Health Supervisor and Women's Health Director. Assist the Medical Oncology team and Behavioral Health Provider in managing care for vulnerable patients across multiple health settings to ensure continuity during transitions of care, with special attention to those having complex medical, behavioral, and social needs. Maintain effective working relationships with patients, families, and co-workers while prioritizing multiple tasks in a fast-paced environment. Requires current BLS certification and one year of medical or behavioral health experience. Bilingual in English and Spanish is preferred.
Required Qualifications
- 1 year of medical or behavioral health experience
- Current BLS through the American Heart Association (AHA) or obtained within 3 months of hire
- Eligible to work in the United States
- Ability to work independently
- Ability to understand and present verbal instructions and to exchange verbal information
- Ability to establish and maintain effective, courteous working relationships with patients/families, co-workers and others
- Ability to work under pressure in a fast-paced environment
- Ability to prioritize and perform multiple tasks
- Ability to work in Windows based computer environment
- Ability to work in professional manner
Desired Qualifications
- Associate or bachelor's Degree in related field
- Significant experience in the behavioral health and/or substance abuse field
- Bilingual (English/Spanish)
- Preferred: Experience working with Substance Abuse issues
- CAC I or higher preferred or willing to obtain
