Job Summary
The Behavioral Care Manager is responsible for providing and coordinating behavioral health care management for patients receiving HIV prevention and care services. This role involves effective communication, timely documentation of services delivered, and participation in case conferences to ensure a team-based approach to patient care. The manager will conduct individual counseling, monitor patient progress, facilitate psychoeducation, and consult with other health professionals. Applicants must have a Master's Degree, at least 5 years of job-related experience, and a NYS-approved mental health license, such as LMSW or LMHC.
Required Qualifications
- Minimum formal education of Master Degree
- Minimum of 5 years job-related experience
- License NYS-Approved Mental Health, including LMSW or LMHC
- Experience providing mental health services to clients who receive HIV services and/or to special populations
- Intermediate computer experience, including proficiency in Microsoft Office Suite
Desired Qualifications
- Minimum certification of a NYS-approved mental health license, including LMSW or LMHC
- 1+ years’ experience providing behavioral health services to underserved communities
- Fluency in Spanish highly preferred
