Job Summary
Provide in-home care services to individuals with challenging behaviors, implement and monitor individual service plans, track data daily, respond to behavioral incidents, coordinate with managers and coordinators, administer medications, assist with activities of daily living (hygiene, peri-care, oral hygiene), and cover Direct Support Professional duties as needed. Role requires 21+ years old, ability to travel in surrounding areas, and strong communication and documentation skills. Job is part-time based in Eagan, MN, with benefits and a supportive clinical team.
Required Qualifications
- High School Diploma, GED, or equivalent
- 1+ years of experience with challenging behaviors
- AA Degree
- Background check (MN DHS)
- Willingness to travel to surrounding areas
- Reliable transportation and a valid driver's license
- Fluently read, understand, speak, and write English
- Dynamic communication skills and multitasking
- General computer skills/knowledge
- Cell phone with internet access
- Experience with Positive Supports and Crisis Interventions
- Reading and following Protocols (e.g., Seizure Protocol, Behavior Reduction Protocol)
