Job Summary

Assist patients, families, and significant others with physical, emotional, and social concerns during the episode of care. Provide treatment and discharge planning services while completing documentation including treatment plans, safety plans, and progress notes. Conduct initial assessments to gather background, social, and financial data, documenting social determinants of health in the medical record. Connect clients with community resources to aid recovery and coordinate care with external providers. Act as a liaison with adult and children protective services as indicated. Cooperate with facility staff and physicians by informing them of patient progress through verbal communication and chart documentation. Provide oversight in situations requiring a licensed individual and offer expert training and supervision to others as requested. Follow policies, procedures, and safety standards while participating in quality improvement initiatives.

Required Qualifications

  • Master's degree from accredited graduate social work or counseling program
  • Five (5) years of experience in a behavioral health related field
  • Must be licensed as LCSW, LPC, or LMFT
