Job Summary
Assist patients, families, and significant others with physical, emotional, and social concerns arising during the hospital stay. Provide discharge planning services and complete required documentation prerequisites for placement and authorizations prior to discharge. Perform initial assessments and continued documentation to obtain pertinent background, social, and financial data, including social determinants of health. Connect individuals with community resources to aid treatment and recovery, while acting as a facility liaison with adult and children protective services. Cooperate with hospital staff and physicians by informing them of patient progress through verbal communication and chart documentation. Follow policies, procedures, and safety standards, completing required annual education assignments and participating in quality improvement initiatives.
Required Qualifications
- Bachelor's degree (BSW) from accredited social work program OR Master's degree in a behavioral health related field
