Job Summary
Coordinate the development of client skills, self-reliance, and relapse prevention while co-creating guidance plans with personal coaches. Support clients in daily tasks such as cooking, shopping, and maintaining their living environment, alongside coordinating appointments with external providers. Contribute to the team's 24-hour coverage by managing day, evening, and intermediate shifts to facilitate social skill acquisition and community participation for individuals with psychiatric vulnerabilities or addiction. Work within a team focused on helping clients transition to independent living after one to two years.
Required Qualifications
- Relevant diploma at MBO level 4
- Demonstrable work experience in psychiatry and/or addiction care (preferred but treated as required qualification in this context due to lack of softening language)
