Job Summary
Conduct daily mental health support for clients with complex psychiatric issues through a hybrid model of on-site care at our mixed housing location in Zevenbergen and ambulatory guidance across West-Brabant. Co-create care and guidance plans with clients while monitoring progress across living, financial, and social domains. Lead the guidance trajectory, manage goals, and coordinate with treatment providers and network partners. Identify risks and take timely action during relapse, stagnation, or crisis situations. Provide individual and group support within the housing setting to foster self-determination and practical skills. Work shifts between 09:00 and 20:00 with flexibility to cover protected housing locations.
Required Qualifications
- een hbo-diploma in een zorg- of welzijnsrichting
- kennis van en ervaring met psychiatrische en psychosociale problematiek
- ervaring met coachende begeleiding en het versterken van zelfredzaamheid
- een rijbewijs B
