Job Summary
Manage capacity and access by eliciting patient information from referring physicians and outside facility nurses to facilitate appropriate triage and placement. Utilize triage tools and algorithms in collaboration with patient access services to transfer patients within clinically appropriate timeframes while acting as a liaison to ensure continuity of care. Coordinate standardized transfer pathways, balance patient flow, and collaborate with providers to facilitate comprehensive decision-making for patient and family needs. Document visit reasons, testing, referrals, and medication lists using established systems. Requires a Bachelor's Degree in Nursing and a Registered Nursing License in Massachusetts; experience in patient flow, access nursing, or bed control is preferred.
Required Qualifications
- Bachelor's Degree Nursing
- Registered Nursing License in Massachusetts
- Clinical nursing experience 2-3 years
- Possesses interpersonal skills to interact effectively with intra-inter and outside-hospital physicians and personnel
- Strong theoretical and experiential knowledge of nursing practices
- Must possess skills to work well independently
- Sound decision-making skills
- Ability to manage critical issues relating to patient care
- Ability to manage critical issues relating to changing organizational climate
