Job Summary
Conduct psychiatric re-assessments, identify physical health needs, and administer medications in conjunction with a psychiatrist while managing shift responsibilities within a multidisciplinary team. Provide treatment, rehabilitation, and support services to participants with severe mental health conditions, ensuring continuity of care and enhancing independence living skills in daily living, social support, and recreational areas. Develop strength-oriented treatment plans utilizing community resources and maintain accurate documentation of all billable services, including medication administration records and personal records. Offer educational and training sessions to participants, families, and provider agencies based on individual and group needs.
Required Qualifications
- Graduation from an accredited school of practical nursing
- Pennsylvania License in good standing
Desired Qualifications
- Experience working with persons with mental illness
- Two (2) years' experience in the mental health field
