BE Case Management Specialist

Job Summary
Assess individual strengths and needs using standardized instruments to operationalize data for each assigned consumer. Collaborate with multidisciplinary teams to coordinate care, link with primary physicians, and maintain connections with community-based health organizations. Develop, implement, and monitor service plans through monthly reviews while documenting all activities on agency forms. Liaise with individuals and service providers via telephone and face-to-face meetings to advocate for changing needs and problem resolution. Accompany participants to appointments for financial entitlements, housing, vocational, and court sites, providing training on public transportation and job-seeking skills. Participate in daily team meetings, clinical care meetings, and discharge planning sessions to address persons requiring extensive services. Perform after-hours duties including weekly on-call rotation and evening/weekend shifts to ensure 24/7 access. Complete required documentation and training in a timely manner.
Required Qualifications
- Bachelor's Degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling or education
- Bachelor's Degree in nursing
- Registered Nurse (RN)
- Two (2) years related experience
- Three (3) years mental health direct care experience
- Expertise in behavioral health
- Expertise in criminal justice system
- Expertise in substance abuse
- Expertise in vocational
- Expertise in geriatric
- Expertise in MR
- Expertise in housing
- Expertise in entitlement issues
- Valid driver's license
- Auto insurance
- Access to a vehicle
- Good verbal/written communication skills
- Good, creative problem-solving skills
- Ability to work independently
- Ability to be flexible/adaptive in handling changing priorities in a fast-paced work environment
- Computer skills
- Able to sit for at least one (1) hour at a time
- Able to climb stairs when visiting participant residences
- Able to lift up to 20 pounds when assisting participants with residential moves
- Able to work in the community, some part of every work day
- Able to work evenings and weekends
- Good knowledge of City transit system
Desired Qualifications
- Bachelor's Degree in nursing with 3 years mental health direct care experience
- Registered Nurse (RN)
- Expertise in behavioral health with one or more of the following: criminal justice system, substance abuse, vocational, geriatric, MR, housing or entitlement issues
- Computer skills preferred
