Job Summary
Provide direct personal care and supportive services to participants aged 55+ in their homes, assisting with activities of daily living, light housekeeping, meal preparation, and condition observation. Work one-on-one under RN supervision to identify and report changes in physical, mental, or environmental status to ensure timely intervention. Collaborate effectively with the Interdisciplinary Team to maintain continuity of care while documenting reports and records accurately. Maintain current CPR certification and a licensed driver's license, adhering to emergency safety procedures and schedule changes based on participant needs.
Required Qualifications
- High school diploma or General Education Degree (GED)
- Must be at least 18 years of age
- Current State Registered Nurse Aide (SRNA) certification through the Kentucky Board of Nursing
- Completion of an approved Certified Nursing Assistant (CNA) training program
- Minimum of one (1) year of experience working with frail and elderly population or must receive appropriate training upon hire
- Food handling permit
- Current CPR certification
- Ability to work independently with minimal supervision and adapt to changing participant needs and schedules
- Demonstrated ability to provide compassionate care and effectively communicate with the adult and elderly population
- Ability to read, understand, and follow written and verbal instructions, policies, and safety procedures
- Ability to complete accurate and timely documentation, reports, and records
- Ability to collaborate effectively with PACE staff and members of the Interdisciplinary Team (IDT)
- Strong interpersonal and communication skills, with the ability to interact effectively with participants, families, and interdisciplinary team members
- Ability to solve routine problems using standardized and sound judgment
- Ability to accommodate schedule changes based on participant needs and program demands
- Ability to follow emergency, safety, and evacuation procedures
- Must be a licensed driver
