Job Summary
Conduct home health visits in Dorchester, Charleston, and Berkeley Counties to perform assessments, review care quality, and evaluate physician treatment plans. Collaborate with patients, families, and interdisciplinary teams to develop, execute, and monitor plans of care while counseling clients on healthcare needs. Complete accurate clinical notes, coordinate equipment and supplies, supervise paraprofessionals, and participate in QAPI programs and emergency preparedness training. Requires one year of home health experience, current RN license, CPR certification, and valid driver's license. Full-time weekend schedule with competitive pay ranging from $78k to $83k, weekly pay, mileage reimbursement, and comprehensive benefits including PTO and 401K.

- Graduate of an accredited school of nursing
- Current RN license
