Baylor RN (Home Health)

Provide in-home nursing care in the Boston, MA area as part of Pathways Healthcare’s Home Health program. Responsibilities include initiating preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed, initiating and revising the plan of care as patient status changes, responding to after-hours calls, assessing patient needs through triage and in-home visits, collaborating with physicians, nurse practitioners and the interdisciplinary team to contribute to a strong plan of care, responding to inquiries from patients and caregivers, and consulting with patients and caregivers before, during and after treatment. Requires MA RN licensure, BLS CPR certification, a valid driver’s license, and reliable insured vehicle; experience with geriatrics and excellent communication is preferred and training is provided for new nurses.
