AWC Program Nurse (RN or LPN)

Job Summary

Conduct medical and psychosocial assessments for all participants during intake and annually, while monitoring physical wellbeing daily through weekly phone calls for non-attenders. Prepare and update plans of care semi-annually, document incidents in nursing notes and participant databases, and manage medication administration, storage, and inventory logs. Perform daily rounds, monitor vital signs, and coordinate emergency responses, injuries, and referrals to community resources. Ensure confidentiality of medical information, maintain stocked first aid kits, and handle sharps disposal in compliance with agency policies. Facilitate staff training, coordinate temporary nursing coverage, and submit monthly service reports under the guidance of the Program Supervisor.

Required Qualifications

  • Minimum Registered Nurse (RN) state credentials
  • Licensed Practical Nurse (LPN)
  • Prior experience working with older adults
  • Must be willing to complete any required onboarding training and ongoing in-service training as needed
  • Must be CPR and RN certified
  • Must be able to provide written copy of active certification/license
  • Bilingual in Spanish and English
  • Computer literacy
  • Ability to competently and routinely use a computer to enter and/or retrieve data
  • Proficiency in the use of Microsoft Office applications including but not limited to: Outlook, Word, Publisher, and Excel
  • Ability to access and use the internet and web-based data sites, when needed
  • College-level math skills
  • High School level math skills
  • Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions and decimals
  • Ability to complete reports and vouchers accurately
  • Ability to read, create and summarize statistical reports
  • Ability to work independently and collaboratively
  • Tact, professional composure and diplomacy when interacting with others
  • Ability to work collaboratively with diverse populations
  • Patience and a supportive demeanor when engaging with AWC participants and their family members/caregivers
  • Ability to demonstrate the ability to work independently in executing tasks, utilizing good judgment and sound decision-making
  • Discernment regarding issues or situations, which warrant supervisory attention
  • Excellence in interpersonal communication
  • Good time management and organizational skills
  • Must be able to travel to other locations throughout the city or state as part of professional development trainings, conferences or other events
  • On rare occasions, this position may require a working lunch
  • Must be able to lift and carry 25lbs
  • Sensitivity to cultural diversity
  • Will be subject to drug and alcohol screening
  • Must be able to perform each essential duty satisfactorily
