Build meaningful relationships while providing guidance, encouragement, and support for residents with identified needs. Assist with daily living activities including personal care, hygiene, meals, household support, and transportation. Support community involvement, social connections, and independent living skills while advocating for rights, needs, and preferences. Monitor health and well-being, reporting changes as needed, and complete required documentation. This residential group home setting supports up to four residents in a collaborative, mission-driven team focused on dignity and inclusion. Positions require 40% FTE or greater, a valid driver's license, and the ability to pass a DHS background check. No prior experience required; training is provided.
