Job Summary

Provide direct patient care in the home setting as assigned, ensuring compliance with clinical and agency standards. Serve as an immediate, reliable resource to cover last-minute callouts, open shifts, and new or vacant cases. Assist with the onboarding and transition of new clients and employees, ensuring smooth service initiation and satisfaction. Provide leadership and field mentorship, assisting with case orientation and skill development of new employees to ensure successful case transitions and retention. Ensure full compliance with Electronic Visit Verification (EVV) policies and accurately document all visit care and activities per company policy and state regulations. Represent Avid positively in the community as a brand ambassador, participating in events, schools, and job fairs. Support recruitment and engagement of qualified field staff through referrals, networking, and outreach activities. Collaborate with HR and field leadership to identify staffing gaps and recommend solutions. Full-time (40 hours per week) with flexible scheduling to meet agency and staffing needs. Monday–Friday, with weekends and holidays as needed.

Required Qualifications

  • Current certification/registration with the Licensing Board or Department in state of practice
  • Current PCA, CNA, HHA, LPN, or RN licensure
  • Must remain in compliance with all regulatory requirements and company policies
  • Must meet transportation, language, and cultural requirements specific to branch needs
  • Must demonstrate a compassionate attitude toward the care of the sick
  • Ability to read, write, and follow directions

  • Hours: Monday–Friday, with weekends and holidays as needed
  • Travel Required: Must have a valid driver's license
  • Must have proof of insurance
  • Must have a satisfactory driving record
  • Must have a reliable vehicle for business use
  • Employee must have the ability to lift, push, pull and carry a minimum of 25 pounds
  • Employee must have the ability to stoop and bend up to 2.5 hours per 8-hour shift

Desired Qualifications

  • Minimum of 1–2 years of field experience in home care, hospice, or healthcare
  • CPR certification
  • Bilingual (English/Spanish)
  • Additional Requirements: Bi-lingual – English/Spanish a plus
