Job Summary
Provide and document skilled nursing care in patients' homes per individualized care plans and physician orders, coordinating with supervisors and physicians. Continuously assess patient condition, implement and update nursing care plans, take appropriate actions to ensure patient safety, and communicate changes. Maintain thorough documentation of assessments, actions, and outcomes; adhere to confidentiality and professional standards; deliver care utilizing infection control measures per OSHA guidelines; educate patients and families on disease processes, self-care techniques, and prevention strategies. Demonstrate attention to detail, time management, problem-solving, and strong communication skills; flexible scheduling includes nights, weekends, and PRN opportunities. Requires RN license in practice state, CPR certification, ability to travel, and capability to work across home health settings.
Required Qualifications
- Active, unencumbered RN/LPN/LVN license in the state of practice
- Current CPR certification
- TB skin test within last 12 months
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Must have reliable transportation
- Six months hands-on nursing experience preferred
- Home health or private duty nursing experience (preferred)
