Job Summary

Provide skilled nursing care and coordinate quality patient care in compliance with physician orders while continuously assessing patient conditions and reporting changes. Document all activities, assessments, nursing actions, and care coordination in a timely manner using electronic documentation tools. Develop, implement, and update nursing care plans, taking appropriate actions based on assessments to achieve expected outcomes and prevent or reduce risks. Educate patients and families regarding disease processes, self-care techniques, and prevention strategies. Maintain knowledge of nursing competencies through continued education and skill evaluations while adhering to infection control measures and professional boundaries.

Required Qualifications

  • Active, unencumbered RN license in the state of practice
  • Current CPR certification (with hands-on component)
  • Current TB skin test (within last 12 months)
  • Reliable transportation
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current, unrestricted state license as a Registered Nurse in the state of practice
  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
  • Ability to speak, write, read and understand English
  • Ability to travel
  • Ability to lift 50 pounds
  • Ability to sufficiently reposition patients and move equipment without assistance
  • Ability to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Ability to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases (implied requirement to function in this environment)

Desired Qualifications

  • Six months prior hands-on nursing experience
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
  • One (1) year of pediatrics experience (South Carolina specific)
  • One (1) year of experience working under current nursing license (California specific)
  • One (1) year of experience working as a licensed nurse (Louisiana specific)
  • Two years Registered Nurse experience with cardiac monitoring and oncology experience (Medical Oncology RN)
  • Two years experience with vital assessment, plan of care development, and telephone triage (Ambulatory RN)
  • Two years Registered Nurse experience (Medical Surgical RN)
  • Two years of Registered Nurse experience, preferably with oncology experience (Peri-Operative Care RN)
  • Attention to detail
  • Time Management
  • Effective problem-solving and conflict resolution
  • Good organization and communication skills
