Job Summary
Licensed Practical Nurse/LVN to provide skilled nursing in patients' homes under the supervision of an RN. Flexible scheduling includes full-time, part-time, or PRN with days, nights, and weekend shifts. Responsibilities include delivering and coordinating quality patient care per physician orders and care plan, documenting activities and nursing actions, updating care plans, infection control per OSHA standards, educating patients and families, and maintaining professional competencies. Requires graduate of an accredited nursing program, active LPN/LVN license, CPR certification, and ability to travel. State-specific requirements may apply; six months of recent home health or clinical nursing experience and/or pediatric experience preferred. Travel and reliable transportation are required. Required Qualifications
- Active LPN/LVN license in state of practice
- Current CPR certification
- TB skin test current within last 12 months
- Six months hands-on nursing experience preferred but not required
- Reliable transportation
