Aveanna Healthcare Private Duty Nurse LVN - Nights & Weekends

Home-based Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under RN supervision per individualized care plans, conducts clinical assessments, updates and implements nursing plans, maintains timely documentation, utilizes infection control measures, educates patients and families about disease processes and self-care, and demonstrates professional accountability while traveling to patient homes across assigned Texas locations (e.g., North Austin, Round Rock, Pflugerville, Giddings). Responsibilities include observing patient condition, reporting changes, coordinating with care team, ensuring compliance with policies and regulations, and participating in ongoing education; requires active nursing license, CPR, and state-specific experience as applicable; travel and home health environment experienced preferred.
