Job Summary
Licensed Practical Nurse (LPN/LVN) to provide and document skilled nursing care in patients' homes under the supervision of a Registered Nurse. Roles include delivering care across nights and weekend shifts in the Austin-area home health setting, with opportunities for full-time, part-time, or PRN schedules. Responsibilities include implementing and updating the nursing care plan, observing patient condition, documenting activities and nursing actions, communicating changes to supervisors/physicians, maintaining infection control, patient and family education, and adherence to professional standards and regulatory requirements. Requires active LPN/LVN license, CPR certification, and compliance with state nursing regulations; travel to patient homes is required. Helpful attributes include strong organization, communication, problem-solving, and time management skills. State-specific requirements may apply (e.g., pediatrics experience in certain states).
Required Qualifications
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- TB skin test (current within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Must have reliable transportation
- Home health experience preferred
