Job Summary

Deliver and coordinate quality patient care in compliance with physician orders while continuously assessing patient conditions and reporting changes to supervisors. Document all nursing actions, assessments, and care coordination in electronic systems, participate in updating nursing care plans, and implement interventions to prevent or reduce patient risks. Educate patients and families on disease processes and self-care techniques, maintain infection control standards per OSHA guidelines, and ensure personal accountability by adhering to state regulations and Aveanna policies. Provide skilled nursing care in home settings for children and adults, utilizing tablet-based documentation tools.

Required Qualifications

  • Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
  • Compact licenses must be transferred to your state of residence within 90 days
  • Current CPR certification (with hands-on component)
  • TB skin test (current within last 12 months)
  • Must have reliable transportation
  • Must be able to speak, write, read and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
  • Must be able to sit and climb stairs
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases
  • Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
  • Must comply with state specific requirements for South Carolina, California, or Louisiana (one year of experience)
  • Continuing Education as required by state
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures

Desired Qualifications

  • Six months prior hands-on nursing experience preferred but not required
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
