Job Summary
Licensed Practical Nurse/LVN providing in-home skilled nursing care under the supervision of a Registered Nurse. Night shifts available with flexible scheduling to accommodate full-time, part-time, or PRN needs. Responsibilities include delivering, coordinating, and documenting nursing care per care plans and physician orders, monitoring patient condition, communicating changes, maintaining accurate documentation, implementing and updating care plans, ensuring infection control per OSHA standards, educating patients and families, and maintaining professional competencies. Requires an active LPN/LVN license, CPR certification, ability to travel to patient homes, and adherence to state nursing regulations.
Required Qualifications
- Active, unrestricted LPN/LVN license in the practicing state
- Current CPR certification
- TB skin test current within last 12 months
- Six months of hands-on nursing experience preferred but not required
- Reliable transportation
- Eligibility to work in the United States
- Continuing Education as required by state (where applicable)
