Job Summary

Deliver and coordinate quality patient care in compliance with physician orders while continuously assessing patient conditions and reporting changes to supervisors or physicians. Document all activities, assessments, nursing actions, and care coordination in a timely manner, and participate in updating nursing care plans to achieve expected outcomes. Implement appropriate nursing interventions based on assessments to prevent or reduce risks, maintain infection control measures per OSHA standards, and educate patients and families on disease processes and self-care techniques. Accept professional accountability by complying with Aveanna policies, state regulations, and the Nurse Practice Act while maintaining knowledge of nursing competencies through continued education.

Required Qualifications

  • Licensed Practical / Vocational Nurse – LPN/LVN
  • Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
  • Compact licenses must be transferred to your state of residence within 90 days
  • Current CPR certification (with hands-on component)
  • TB skin test (current within last 12 months)
  • Must have reliable transportation
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Must be able to speak, write, read and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases
  • As an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate

Desired Qualifications

  • Six months prior hands-on nursing experience preferred but not required
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
  • Attention to detail
  • Time Management
  • Effective problem-solving and conflict resolution
  • Good organization and communication skills
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
