Job Summary
Licensed Practical Nurse (LPN/LVN) providing and documenting skilled nursing care in patients' homes under the supervision of a Registered Nurse, per individualized care plans and physician orders. Responsibilities include monitoring and assessing patient condition, timely documentation of nursing actions, updating and implementing care plans, educating patients and families, ensuring infection control per OSHA standards, and coordinating care with the care team. Must maintain an active, unencumbered LPN/LVN license in the practicing state, current CPR certification, and meet state-specific requirements. Flexible scheduling available including days, nights, and weekends with travel to patient homes; 24/7 local support and opportunities for skill advancement.
Required Qualifications
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice (LPN/LVN)
- Current CPR certification
- TB skin test (current within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
- Ability to travel
- Continuing Education as required by state
